2009 CFT Surveys
The Durham System of Care Outcome Workgroup is coordinating our regular review of Child & Family Teams during the period of October 12th – November 13th. All completed CFT Survey packets must be returned by November 20th.
It is very important that you read the instructions so that your questions can be addressed before implementation. We are happy to assist you in this process and to provide further guidance or training in the survey administration. Specifically, we want to know how many teams are in practice, what the strengths and struggles of those teams are, and where gaps exist that might be better supported.
This process is designed to assist us all in serving our consumers with the highest quality of services and to give us a picture of how we are doing as a community with the implementation of Child and Family Teams. Survey data is collected by Private Providers, Public Schools, Dept. of Social Services and the Dept. of Juvenile Justice and Delinquency Prevention. The data will be summarized in the end of year CFT Report. The survey data is grouped by agency type rather than by single agency providers.
Your participation is very important to our continuous quality improvement efforts to better meet the needs of our community. Again, the survey period is October 12th – November 13th and all completed CFT Survey packets must be returned by November 20th. Please contact us with questions.
