Recognized Partner

Agencies and organizations must submit an application in order to be considered for participation. (Support for completing the application process is available, if needed.) All organizations seeking to become a “Recognized Partner” will be required to sign a Memorandum of Agreement and must agree to:

  Develop and execute a CFT Handbook Implementation Plan, agree to Plan reviews and verification of implementation;

  Actively participate in the Leadership Roundtable;

 Participate in the work of the Community Collaborative and at least one of its workgroups;

 Provide detailed outcome data about Child and Family Teams to Durham System of Care evaluation staff; and,

 Participate in DSOC quality review processes, including CFT observation and document reviews.
