Free Emergency Card

Create a Free Emergency Card . Choose the link for the Durham Network of Care website and login or create a new account. Then you will need to click on "Primary Folder" on the left of the screen, click on "Make New Emergency Card", click on "Create My New Emergency Card", fill in the form, then click "Send Me My Laminated Card". You will recieve your card in the mail in approximately 2 weeks. You may print a copy for your current needs as well.

Your Emergency Card lists critical information that rescuers need to know if they find you unconscious or need to quickly help evacuate you. Make multiple copies of this card to keep in emergency supply kits, emergency carry-with-you kits, car, work, wallet, purse, or wheelchair pack.
