Continuous Quality Improvement Plan

Overview

Durham System of Care (DSOC) quality assurance process is a county level effort seeking to determine how Child and Family Teams (CFTs) are doing in meeting the needs of children and families. This is an on-going assessment of practice, resource utilization and most importantly child and family specific outcomes. DSOC has developed a Logic Model to guide this process.

The Outcome Workgroup, a workgroup of Durham’s Community Collaborative, comprised of agency personnel, parents, and private providers among others meet regularly to provide feedback to this process. This collaborative process will also allow the development of evaluation capacity across partner agencies and the provision of information from multiple sources for program improvement.

Partner agencies include: The Durham Center and private providers, Durham County Department of Social Services, Durham County Department of Public Health, Durham Public Schools, Department of Juvenile Justice and Delinquency Prevention, Durham District Court, Durham County Cooperative Extension.

Review of Child and Family Teams

The goal of this quality assurance process is to assess the quality of Child and Family Teams (CFTs) occurring within the Durham System of Care (DSOC) partner agencies that include Durham Public Schools, The Department of Social Services and Private Providers/LME.  The review of CFTs include CFTs structure and process and satisfaction with services as rated by team members.  Methods of data collection include surveys and face to face interviews.  The CFT surveys have been developed for use by any potential participant in a CFT (e.g., parents/caregivers, school counselors, social workers, agency staff / service providers, natural and personal supports, and advocates). Some items have been used with authorization and courtesy of the following sources: 1) Center for Child and Family Policy-Duke University. Federal Grant # 90-CA-1711. Department of Health and Human Services, 2) Wraparound Fidelity Index 4. University of Washington/ Copyright 2005 Wraparound Evaluation and Research Team.  Brevity and readability were also considered in the instrument development process. A Spanish version of the CFT survey is available.

Support for DSOC Initiative in the Durham Community

An evaluation of the Durham System of Care (DSOC) Initiative as it relates to the development of a multi-level network of agencies will be included in the fiscal year 2007/08. A data collection tool from the National Wraparound Project will be ulitized for this area of inquiry.
