What is Durham System of Care?

The Durham System of Care (DSOC) is an organizational framework, an approach, a way of 'doing business' driven by values and principles that enables our community to better meet the needs of individuls, children and families. It is not a program but a way to use teamwork to organize and support a coordinated aray of community services and resources.

How does System of Care work in Durham?

  • At the heart of our System of Care is a shared commitment to help individuals and families in meeting their goals through family involvement and teamwork.

    • For the individual or family,

      •  Teams, called Child and Family Teams or Recovery Teams, are comprised of people important in the everyday lives of the individual or family work together to identify strengths needs and plans to meet their desired goals.
    • At the community level,

      • The Community Collaborativeis a team of representatives from community human service agencies (public and private), public schools, along with individuals, family members and other stakeholders.
      • They work together to plan, evaluate and improve our community

  • If asked to participate on a Child and Family Team or Recovery team, say yes
As an existing or new human service delivery program:
  • Actively contribute by sharing resources and expertise as a member of the Community Collaborative
  • Deliver your services through Child and Family Teams or Recovery Teams
  • Be accountable to Durham's System of Care common outcomes
